1FTFW1E50MFA34297 21 Lariat 5.0. Purchased in early November. Stuck on 4.2.3 for almost the entire time. Just noticed today it has 4.2.4. Is it typical to just download that one little update? If connected, won't it do the OTA until current?

If the truck was sitting on a dealer lot it won't update until it's turned on, so any updates queued were missed. They get put back in queue on random OTA schedule. If you've seen an update happen Ford will eventually send more. You aren't missing out on features at all and in your shoes I wouldn't bother with FDRS. You do have updates coming up after where you are that can be difficult to get through so have a good battery. If they fail you'll get a notice.

I don't think I'm missing out on anything. There are just some odd things the Sync does. For instance, I get a text and it reads aloud to me, the source is then set to my phone and not the FM it was before the text came in. I just thought some of these updates might be little patches or fixes.

It will sound silly but a large number of crazy sync bugs are resolved setting it to use dark mode full time for the screen. Give that a try if not already using it.

Excuse me as I am a little green as to what this is telling me. Are all these updates "available" and need to to be loaded on the vehicle or will all these updates be downloaded once the OTA's reach 6.8.xUpdates available:

Some of these could potentially "fix" some quirks you might be having. These updates can be applied to your truck with a Mongoose cable and a FDRS subscription. The cable is a costly investment, but there is a thread to get on a waiting list to borrow one if you didn't want to purchase one. Some of these updates potentially could come to you via OTA, but that is hit and miss and anybody's guess as to when it may happen.
