    Only difference I'm aware of is you'd need one from the same wheel base as you have, as the 6.5' crew cab has a 2-peice drive shaft and the rear end has a CV joint, where the 5.5' trucks have a single peice drive shaft with the typical U-joint on the rear axle.

    With the engine cold, you'll want to check and verify that your coolant level is 'full' seems like you are maybe not circulating enough coolant through the heater. I can't think of any other reason the heater would not be as warm at idle vs higher engine speeds....
