My rear axle was fairly rusty when I bought my '22 F-150 used a year ago. I got under there and wire brushed the surface to remove loose rust and then coated it with Corroseal rust converter using a sponge brush. It turned to a nice black finish. Didn't even need to cover with paint. So far it's holding up nice, still looks like the day I refinished it. I do drive on salted winter roads, so we'll see how long it lasts.
In my experience paint on top of rust will start to flake off in time.
