I took my truck back in after the latest TSB failed to fix dead pedal. The service technician wrote a snarky statement that the transmission is working as designed and that I don't know how the transmission works on a hybrid truck.

I am back with an update. I have had the 2nd gear issue occur with the truck warmed up. They just did TSB 25-2302. As "luck" would have it, I stopped off at a store, drove out of the parking lot with a right turn and the dead throttle occurred. This was on the way home from the service...
My truck went in for this recall and the issue remains. The Sync 4 crashes as frequently as it did before. I have an active dog who was playing with the kids and he ran for the ball as I was backing up. The image froze on the camera right at the worst possible time.

I'd love to know if we are all on the same version of software and chips. Do the overhauls get new control software/hardware? Only Ford knows, and they aren't focused on this.
If the issue persisted in every mode (Normal/Tow/Eco), then we'd be looking at a mechanical problem. Since the mode stops the issue, it's not a surprise that the valve body didn't solve anything.
Just to update, I can reproduce the issue at will in my 2021 PB, but only when the truck is in Normal mode. If I switch to Tow/Haul or Eco mode I cannot reproduce the issue. I haven't tried any other modes, such as Sport. 2nd Gear, cold truck, Normal Mode induces the problem.
This mystery Ford account surfaced in post #5, with no subsequent response, to your point. The official Ford account seems to be more active in that other F-150 website and I have observed their intervention with positive results in the past. But, it's crickets here...
I have never had the truck fail to accelerate when it is fully warmed up. Since I determined 2nd gear, accelerating, when cold, allows 100% reproduction I am hyper focused on what is possible/not possible in traffic.
I am not a transmission tech, but my thinking is a mechanical failure issue...
My 2021 F-150 XLT PB does this as well. I can replicate the issue at will as I've posted in the thread
